On or about March 14, 2016, two employees got into an altercation while rinsing at the wash sink. The first employee took a knife and meat hook and struck the second employee, lacerating his clavicle area and neck. The first employee suffered a laceration to the side of his face.

An employee was waiting for maintenance to clear a jam on a tissue interfolder machine. The employee attempted to grab a scrap of paper as maintenance was jogging and stopping the machine. Her right pinky and ring fingers were pulled into a pinch point and had to be surgically amputated.

An employee climbed an 8- to 10-foot ladder to open the door to a confined space. The door was under pressure, and as the employee opened the final latch, it came open and knocked the employee from the ladder. He fell approximately 8 feet, fracturing his skull, breaking several ribs, and puncturing a lung.

On February 14, 2016, an employee was scrapping down the sides of a 56 gallon mixing tank with a wooden paint stick. His shirt got tangled with the rotating mixer shaft and the employee was pulled against the tank. The employee suffered a punctured lung, cracked ribs and lacerations to his right side, forearm and forehead.
